A major problem still exists re "public's expectations" and reality. The public expects "highest quality care---regardless of cost" while politicians, employers, and insurers (managed care and otherwise) are very concerned about cost. MDs get caught in the middle and cannot deliver on the politician's promises. At some point someone needs to tell everyone that reality is there must be reasonable limits. (Even now mixed message continues as we try to add prescription drugs to an already overspent Medicare system--where will additional money come from?) We need to make the patient a partner in the health care delivery process -- including a financial incentive for "good health behavior" i.e. you cost less, you pay less with understanding that patient will share in overall money/cost savings.
